About Endometrial Biopsy

An endometrial biopsy is a common, minimally invasive diagnostic procedure performed by an Obstetrician-Gynaecologist to evaluate the lining of the uterus (endometrium). During the procedure, a small sample of endometrial tissue is gently collected using a thin, flexible tube (pipelle) inserted through the cervix. This sample is then sent to a laboratory for microscopic analysis. It is a crucial tool for investigating abnormal uterine bleeding, postmenopausal bleeding, or screening for endometrial cancer and hyperplasia. The procedure is typically quick, performed in an outpatient clinic setting, and provides vital information to guide further treatment and management of gynaecological conditions.

Key Highlights

Minimally invasive outpatient procedure with no need for general anesthesia.Provides a definitive tissue diagnosis for abnormal uterine bleeding.Key screening tool for detecting endometrial cancer and precancerous conditions (hyperplasia).Quick procedure, usually completed within 5-10 minutes.Helps in evaluating infertility causes and monitoring hormone therapy effects.Generally well-tolerated with minimal recovery time needed./ul

Who is this surgery for?

  • Abnormal uterine bleeding, especially in women over 45 or postmenopausal.
  • Investigation of heavy, prolonged, or irregular menstrual periods (menorrhagia, metrorrhagia).
  • Screening for endometrial cancer or atypical endometrial hyperplasia.
  • Evaluation of infertility to assess the uterine lining (endometrium).
  • Monitoring the effects of hormone replacement therapy (HRT) or tamoxifen.
  • Follow-up for previously detected abnormal endometrial cells (e.g., from a Pap test).
  • Diagnosis of chronic endometritis (uterine lining infection).

How to prepare

  • Inform your doctor about all medications, especially blood thinners (aspirin, warfarin).
  • The procedure is ideally scheduled when you are not menstruating.
  • You may be advised to take a pain reliever (like ibuprofen) about an hour before the biopsy to minimize cramping.
  • No fasting is typically required for a simple office biopsy.
  • You may be asked to empty your bladder just before the procedure for comfort.
  • Discuss any history of bleeding disorders or pelvic infections with your doctor.

Risks & possible complications

  • Common: Mild to moderate cramping (similar to menstrual cramps) and light spotting or bleeding for a few days.
  • Infection: Rare risk of pelvic infection (endometritis), signaled by fever, foul-smelling discharge, or severe pain.
  • Perforation: Very rare accidental puncture of the uterine wall, which usually heals on its own but may rarely require monitoring.
  • Vasovagal Reaction: Some patients may feel lightheaded, dizzy, or faint during or immediately after the procedure.
  • Excessive Bleeding: Uncommon, but heavy bleeding (soaking a pad per hour) should be reported.
  • Inconclusive Sample: Occasionally, the tissue sample may be insufficient for diagnosis, requiring a repeat procedure.

Recovery & hospital stay

  • You can usually resume normal activities, including work, the same or next day.
  • Use sanitary pads (not tampons) for any light bleeding or spotting, which may last up to a week.
  • Expect mild cramping for a few hours; over-the-counter pain medication can help.
  • Avoid sexual intercourse, douching, and using tampons for at least 3-5 days or as advised by your doctor.
  • Contact your doctor immediately if you experience fever, chills, severe abdominal pain, or heavy bleeding.
  • Bathing is allowed, but avoid swimming pools or hot tubs for a few days.
  • Follow up with your doctor to discuss the biopsy results, typically available within 1-2 weeks.
  • checked Typical hospital stay: 0 days (Outpatient procedure)
  • checked Expected recovery time: 1-2 days

PATIENT REVIEW

Rahul Brown, a 42-year-old transgender man,...

Rahul Brown, a 42-year-old transgender man, had been on testosterone therapy for 15 years. He had not had a period in over a decade but recently experienced several episodes of light spotting. As a man with a uterus, his complex medical history and dysphoria around gynecological care made him deeply anxious. His primary care doctor, aware of the increased risk of endometrial changes with long-term testosterone use, gently recommended the biopsy to rule out abnormal cell growth. The procedure itself was physically uncomfortable and emotionally taxing for Rahul, requiring significant mental preparation and a supportive, gender-affirming care team. Recovery involved mild cramping for a day. The biopsy results came back benign, showing simple atrophy, which was a huge relief. Emotionally, he felt a mix of vulnerability for undergoing the exam and profound gratitude for a care team that respected his identity, ultimately feeling empowered for prioritizing his health despite the distress.
