About Liver Banding

Liver banding, also known as endoscopic variceal ligation (EVL), is a minimally invasive endoscopic procedure performed by a gastroenterologist to treat enlarged veins (varices) in the esophagus or stomach, a serious complication of liver cirrhosis and portal hypertension. During the procedure, a flexible endoscope is passed through the mouth to the target area. The gastroenterologist then places small elastic bands around the varices, which cuts off their blood supply, causing them to shrink, scar, and eventually fall off. This effectively prevents life-threatening bleeding or stops active bleeding. It is a critical intervention for managing portal hypertension and is often preferred due to its high success rate and lower complication profile compared to older surgical methods.

Key Highlights

Minimally invasive procedure performed endoscopically, avoiding open surgery.Highly effective in preventing and stopping life-threatening variceal bleeding.Typically performed under sedation, minimizing patient discomfort.Lower risk of complications compared to surgical shunting procedures.Often allows for a shorter hospital stay and faster recovery. Can be repeated as needed for recurrent varices./ul

Who is this surgery for?

  • Primary prophylaxis (prevention) of bleeding from large esophageal varices in patients with cirrhosis.
  • Active bleeding from esophageal varices as an emergency treatment.
  • Secondary prophylaxis to prevent re-bleeding after an initial variceal hemorrhage.
  • Gastric varices (specifically GOV1 type) in certain cases.
  • Patients with portal hypertension who are not candidates for or have failed beta-blocker therapy.

How to prepare

  • Comprehensive evaluation including blood tests, imaging, and endoscopy to map the varices.
  • Patient must fast (no food or drink) for 6-8 hours before the procedure.
  • Review and adjustment of current medications, especially blood thinners, as directed by the doctor.
  • Informed consent process detailing the procedure, risks, and benefits.
  • Arrangement for sedation and post-procedure monitoring.
  • Treatment of any active infection or correction of significant coagulation abnormalities beforehand.

Risks & possible complications

  • Post-procedure chest pain or discomfort and difficulty swallowing, which is usually temporary.
  • Risk of ulceration or bleeding at the banding site, though this is part of the intended healing process.
  • Perforation (a tear) of the esophagus, which is rare but serious.
  • Infection or fever.
  • Reaction to sedation medications.
  • Recurrence of varices requiring repeat banding sessions.
  • Stricture (narrowing) of the esophagus from repeated treatments.

Recovery & hospital stay

  • Immediate recovery in a monitored area until sedation wears off (a few hours).
  • Patients may experience a sore throat and mild chest discomfort for a few days.
  • A soft or liquid diet is recommended for the first 24-48 hours post-procedure.
  • Avoidance of strenuous activity, heavy lifting, and NSAIDs (like ibuprofen) for a specified period.
  • Follow-up endoscopy is typically scheduled in 2-4 weeks to assess healing and place additional bands if needed.
  • Long-term management of underlying liver disease and portal hypertension with medication and lifestyle changes is crucial.
